As LA County officially moved into the ‘red’ tier today, indoor athletic activity can commence for some sports. All athletic activities that are conducted indoors will require weekly COVID testing.
Walnut Valley Unified School District is bringing on Brea Urgent Care to help facilitate this requirement set forth by LA County and State Youth Sports Guidelines.
Every Monday Brea Urgent Care will be set up on our school parking lot to administer the PCR test. This will be a drive through, so please stay in your vehicle at all times. Please look at the link of the map and follow the route.
To expedite the process and to respect your time, a few items are needed:
- Complete this on-line registration (IF you have not created one) www.mobilecoronavirus.com/backtoschool/
- Upload your child’s insurance card
- If your child does not have insurance, please let Brea Urgent Care personnel know upon arrival
- Photo ID
- Complete the Authorization Form and bring it with you each time you test. Authorization Form
If you do not have the above items completed, you will need to complete them in the parking lot.
PLEASE MAKE SURE TO BRING YOUR ID, INSURANCE CARD, and Authorization FORM
Additionally, each day your child is on school grounds for practice/game/or activity, a DAILY HEALTH SCREEN must be completed by a parent.
